No More Room in Hell 2 Infection System Explained

Infection in NMRIH2 comes mostly from zombie grabs — the chance rises as your health drops, and a failed grapple or a grab with no stamina infects you outright. It has two stages: a silent incubation, then an onset of coughing, visual distortion and hallucinations that ends with your Responder rising against the squad. The only cure is Experimental Gene Therapy (one per match); Phalanx pills only delay it. Everything below is taken from Torn Banner's own update notes; where no official number exists, we say so.

Two numbers you will not find here: the exact infection percentage and the time it takes to turn. Torn Banner has never published them for NMRIH2, and the figures on the old NMRiH wiki describe the first game. Anyone quoting a precise percentage for the sequel is guessing.

What infects you

Practical reading: stamina is your infection armour. Grabs are only survivable while you have stamina for the mini-game, so stop sprinting into rooms at empty stamina — the Marathon Runner and Athlete skills matter here as much as in a chase. Stay above half health whenever you can, because under 50% every hit becomes a dice roll.

The two stages and how you find out

  1. Incubation. For a short period the virus incubates and there are no signs that you are infected — by design. If you were grabbed at low health, assume the worst and look for pills.
  2. Onset. Coughing comes first, and since the February 2025 update your squadmates hear it too. Early on you get migraine-aura style visual sensitivity; over time the visual and auditory distortion grows, with audio hallucinations added in several updates including 1.0. Your infection status now shows on the HUD (even in Nightmare's minimal HUD and during mini-games), and the Phalanx use prompt only appears while you are infected. Veins appearing on screen means you are close to turning.
  3. Turning. Your dead body rises against your allies as a zombified Responder with its own voice lines; your scoreboard status becomes Deceased. On Hard and Nightmare that is permanent unless a Rescue Beacon protects you; on Beginner and Normal, permadeath only applies if the team wipes before a respawn point or you die after extraction has been called.

Bandages have no documented effect on infection. Since 1.0 the team's decision tree is simple: Gene Therapy if anyone has found it, pills to stretch the clock, then extract — and if extraction is out of reach, self-sacrifice before you become the squad's problem. Acquired Immunity is the reason it sits at the top of our perks tier list: it is the only skill that reduces the roll itself.

How do you know you are infected?

How do you cure infection in NMRIH2?
Experimental Gene Therapy is the only cure and only one spawns per match: use it, wait roughly 30 seconds, and you are cured and immune. Phalanx pills (3 doses per bottle) only delay and reset the infection. Otherwise, extract before you turn, or self-sacrifice so you do not rise against your squad.
How long does infection take to turn you?
Torn Banner has not published a timer for NMRIH2. The '120 seconds / 320 seconds' figures on the Fandom wiki are for the original No More Room in Hell, not the sequel. Treat the vein overlay as your last warning.
What is the infection chance?
No official percentage has been published. The official rule is directional: the lower your health, the higher the chance from grabs and from ordinary hits once you are below 50% HP; low stamina makes grabs an automatic infection.
Did 1.0 change infection?
Not mechanically. The 1.0 Armageddon notes add new audio hallucinations and change the skill pool (Averaging, Guts and Teddy Bear removed; three reloading skills added) but list no infection balance changes. The last official mechanical description is the 1.0 launch blog plus the Early Access updates cited on this page.

Fewer grabs means fewer rolls: the weapons tier list covers the one-tap headshot guns that keep zombies off you, and the maps page notes which levels put you in Shambler-cloud territory. Difficulty and respawn rules changed in the 1.0 Armageddon update.
